WeCare… Because You Do^℠ is an individualized care coordination program that helps to answer your toughest questions, connect you to the services you need . . . and bring you peace of mind.

How Does WeCare work?

You’ll be connected with your personal Care Consultant who will get to know you, understand your individual situation and help you find solutions to your needs.

You’ll talk with your Care Consultant by phone, email and online at times convenient to you. From your very first call, they’ll start to identify your most important needs and immediately begin to find the services, resources and information you need.

WeCare helps with the big things:

Finding quality services and resources

Planning for long-term care

Addressing financial and legal concerns

Exploring housing options

Understanding respite opportunities

And with all the personal things, too:

Creating strategies for challenging conversations

Finding ways to bring family members together around care

Why is WeCare so effective?

Your Care Consultant follows a proven process that breaks down stressful responsibilities into simple, practical steps. This is called your “Action Plan.” What may have seemed insurmountable before now seems manageable, even doable. Your action steps will guide you toward answers and solutions.

With WeCare, you can still be YOU

With your Care Consultant in your corner, you’ll find there’s room to enjoy your time together with your loved one again, too. Your Care Consultant will make sure both of you are making time for the activities that are important to you.

Better yet, many people find they can enjoy the relationships they once had, prior to assuming a caregiver role.

Start Your WeCare Journey

1 Relief begins with your first call

From your very first conversation, you’ll appreciate the knowledge and support that comes with talking to your own personal Care Consultant. You’ll agree on what’s most important to address first and get started right away.

2 Get the services you need most

Your Care Consultant will begin by getting you connected to the resources you need most. Maybe it’s transportation, in-home help, respite services or home safety. You’ll also get information about what keeps you up at night, like estate planning, financial assistance or insurance procedures.

3 More control, less work

Next comes your Action Plan. With all the tasks sorted into a visible, organized list, the weight starts to lift off your shoulders. Feel free to involve as many friends and family members in this process as you’d like—the more the merrier.

4 Focus on your well-being, too

With so much emphasis on providing care, it’s easy to put your own healthcare on hold. Your Care Consultant will help you find ways to address your own physical, emotional and mental health needs. Even small activities can make a big difference in your outlook!

5 Space to relax

As some of your stressors are effectively managed, take a little time to relax. Enjoy each other. Enjoy what you love to do.
